Send to the below email address the following five separate pieces of information. Please ensure that each piece of information listed below is on a separate line, and enter "DASA TEACH" as the subject heading of your email.

1) Last name, 2) First name, 3) SSN (last 5 digits), 4) DOB (Date of Birth), and 5) Home address (include zip code).
